Integrated Care Process

In compliance with the National Strategy for Quality in Health and the National Health Plan 2012-2016, with the aim of offering citizens high quality healthcare, the Directorate-General of Health, through the Department of Quality in Health, launches Integrated Care Processes.

Integrated Care Processes should be gradually implemented throughout the National Health Service. These apply to different levels of healthcare, and are particularly useful in the healthcare process of people with chronic diseases and/or with certain acute diseases. These Integrated Care Processes require the commitment and articulation of clinical, medical and nursing directors, hospital services, primary healthcare and long-term integrated care, as well as the involvement of other healthcare professionals essential for its implementation.

The documents below present the Portuguese version of the Integrated Care Process for type 2 Diabetes, adjusted and updated in active cooperation with the National Programme for Diabetes.
